Re-Entry Permit at BKK airport

I am aware that I need to provide photocopies of my passport, TM-8 form and a 4cm x 6cm recent photograph of me. Though I have some questions which can’t be answered by looking at this subreddit.

1. If I am on a Non-immigrant ED Visa, are there any extra documents I might need to obtain the re-entry permit?
2. Can I just bring my passport and let them handle the pictures / paperwork?
3. If my flight is at 10:50am at BKK airport, how early must I get to the airport to complete the re-entry permit paperwork and check-in?
