Any recommendations for physical exam (short notice)?

Hello,

I am looking for recommendations in Bangkok of medical facilities to get a full physical exam done (including blood/urine/ECG tests), at relatively short notice.

I need to have the exam performed for a job opportunity abroad, with a form filled out by the doctor in English. Unfortunately, the timeframe I have is very short.

I am male, mid-40s.
