How do older ex-pats manage major, ongoing health problems?

Say you move to Thailand 20 years ago. Now you are 65 and are diagnosed with stomach cancer, god forbid. Your chances of living 10 more years are about 50/50. If you get the right treatment and some luck you can extend to 75. If you have the wrong luck you are dead in 18 months. So my question is – do you stay in Thailand to get treatment for this? Do you go back to Australia/US/UK/etc? What have you seen people in a situation like this do?
